Enrolment

Thank you for your interest in applying to enrol your child at Shelley Primary School. Please note that completing an enrollment package and meeting with school staff does not guarantee you will receive a place at Shelley Primary School.

Please follow the enrolment procedures below:

Complete the Enrolment Package, including both the Application for Enrolment form and Enrolment form and email to Shelley.PS@education.wa.edu.au 

Once the Enrolment Package and supporting documents have been received, one of our staff will contact you to schedule an appointment.

Please ensure to bring original documents such as:

  • Birth Cert, Passport and Citizenship (if applicable)
  • Immunisation History Statement  (issued within two months of the enrolment)
  • Copies of any medical/healthcare action plan
  • School Reports
  • Visa/VEVO including Visa Grant Number  (if applicable)
  • Proof of Residence (provide 3 pieces of evidence)
  • Court order papers/custody papers (if applicable)

Proof of residence

Check the intake area map to ensure you are in the Shelley Primary School catchment area. When completing the Application for Enrolment Form you will also be required to provide 3 pieces of evidence that you reside within the local intake area for Shelley Primary School. Applications cannot be considered until such time as documented evidence is provided to the school. Evidence can be in the form of lease agreement, rates notice, utilities invoices eg. water, power.

Birth Certificate and Immunisation requirements

You will also be required to provide the student’s original Birth Certificate and current Australian Immunisation Register (AIR) immunisation history statement.  Click here for further information on how to obtain a statement. A current statement is one that has been issued within two months of the application to enrol. If your child had vaccinations overseas, you should get them added to the Australian Immunisation Register (AIR): Click here

School Reports

Please provide the latest school and NAPLAN reports for your child.

Overseas students

Students born overseas must hold either; an Australian passport, citizenship papers or appropriate visa before applying to the school. Students who do not hold an Australian passport during the application process must present their Visa Grant Form and passport at the time of enrolment.
